Earthquake and Emergency Forms

These forms need to be updated annually.

To check your forms from last year and make sure the information is still current, please see the Internal Membership Chair.

These forms are duplicates of some of the pages in the Admission Packet, so if you are a new family this year, you do not need to fill out the forms again.

The Red Cross recommends that you create a home emergency kit that includes a grab-and-go backpack for each family member. Including copies of these forms in a folder with your kit would be one easy way to store them at home.

• Earthquake / Emergency Reunification Plan

• Emergency Information Form

Your child’s school emergency backpack must be refreshed annually. Check clothing sizes and update the forms over the summer—your child cannot be admitted to school unless his backpack is up-to-date.

• Personal Backpack Checklist for Child

Putting together your own kit at home? Here are some resources that may be of use:

• SOS Survival Products, Van Nuys. (818)909-0131

Check with the Emergency Preparedness parent to see whether there are any coupons that benefit the school when shopping at SOS.

• Red Cross Page for organizing your kit

• Community Emergency Response Team — free training through your fire station for all types of emergencies, and earthquakes.

• The Great Shakeout — California’s all-state earthquake drill every October

Note that many of the survival foods and MREs offered in prepared kits contain soy, dairy, wheat, corn or other allergens. If your child is unable to eat these foods, you may wish to offer alternatives in his personal kit at home and school. Note that alternative bars, such as Kind bars, may need to be updated several times throughout the year, rather than annually.
