These forms are duplicates of some of the pages in the Admission Packet, so if you are a new family this year, you do not need to fill out the forms again.
The Red Cross recommends that you create a home emergency kit that includes a grab-and-go backpack for each family member. Including copies of these forms in a folder with your kit would be one easy way to store them at home.

Your child’s school emergency backpack must be refreshed annually. Check clothing sizes and update the forms over the summer—your child cannot be admitted to school unless his backpack is up-to-date.

Note that many of the survival foods and MREs offered in prepared kits contain soy, dairy, wheat, corn or other allergens. If your child is unable to eat these foods, you may wish to offer alternatives in his personal kit at home and school. Note that alternative bars, such as Kind bars, may need to be updated several times throughout the year, rather than annually.
